Catholic Charities, Diocese of Fort Worth, Inc. has selected Prime Capital Investment Advisors, LLC, doing business as Qualified Plan Advisors (QPA), to provide the “Model Service” described in the Model Service Agreement, which is made available for Plan Participants in the Catholic Charities, Diocese of Fort Worth, Inc. 403(b) Plan and will be administered through the Plan’s recordkeeper.
